For those of you who are in charge of textbooks, I have created a form on google docs that might be helpful when distributing books to your staff. I hope it helps...I also hope this link works.

The answers from the form are stored automatically in a spreadsheet...this is just a sample...you will have to create a google account and copy all of this to your own account...I can try to help if you need it. Here's the link to the sample spreadsheet:
